01 · Choose the first team
Start with one location, a supervisor and colleagues from different shifts. Choose one routine that currently causes repeated questions.
02 · Configure the handover
Decide the days, shifts and checks. Keep points concrete and short. Agree when an unfinished item needs a follow-up task.
03 · Agree how you communicate
Use task questions for context, team chat for conversation and updates for working agreements. Keep client and medical information in the appropriate systems.
04 · Try it during real shifts
Include temporary staff and less confident phone users. Look at whether the next shift understands the open work, not just whether someone opened the app.
05 · Evaluate before expanding
Ask what people no longer need to search for, where confusion remains and which routines should change. Expand only when the team benefits.
